  • Re: Source for PHS 398 electronic forms?

If you can send me the PDF form of PHS 398, I can try to convert it into a
fillable PDF form and have it available for anyone to download. You will
however, only be able to save your input if you have the full version of
Acrobat, not just Acrobat Reader. A workaround is to create your text in a
word processor then cut and paste into the form when you are ready to print.

For anyone who is interested, I have PHS 6246-1 (SBIR Phase I) and 6246-2
(SBIR Phase II) available for download at
    http://www.isolns.com/phs6246-1.htm and http://www.isolns.com/phs6246-2.htm,
    respectively.

Anyone know of an on-line source for PHS 398 (NIH, etc.) electronic forms?
By "electronic forms", I mean ones where I fill in the blanks on my
    computer,
then have the filled form printed out - save the secretary some typing.
I did find the TRAM/Baylor ones (Thanks, guys!) but they need a little
tweaking so am looking for alternatives.
