search for assessment questionnaire

I am searching for a subjective assessment questionnaire to qualitatively
assess patients before and after finger joint replacement primarily
due to rheumatoid arthritis. Apparantly there are some standard
questionnaires which have been self-tested for validity/reliability, to
make sure that the questions are not leading or unfair. Does anyone know
of any questionnaires which assess hand function, strength, range of
movement, pain, stiffness etc. ( ideally for arthritic
patients) which may be suitable, or who I could contact for further
information.
