After following several email addresses and links which I received from
various people I located a source from which I can obtain a copy of this
thesis. For those of you in the US you can most likely get an interlibrary
loan from UCLA Biomedical library. For those of you outside, or anyone who
wishes to own a copy, go to http://www.umi.com/ then to 'Dissertation
Express' where you can purchase an unbound hardcopy for $24.95 (US).
