Software for MR image segmentation?

Is anyone aware of any software that will do
segmentation from MRI data (2D/3D) and extact
(make available) geometrical information
(i.e. provide representation in (cartesian?)
coordinates of the structures of interest).

Any info / pointers will be greatly appreciated.
