It is long out of print (the original hardcover is going for $1,600 on Amazon!) but "Muscles Alive" (Basmajian and De Luca) is a fantastic text on EMG.
This paper also gives a nice summary of various issues related to measuring and interpreting EMG:

There are several very good chapters covering EMG in the human gait environment in "Gait Analysis:Theory and Application" by Rebecca Craik and Carol Oatis - reading these will give you a basic understanding of EMG. While the book deals with human gait, the discussions will give you a very good background for further study. If you are just starting to learn about EMG then these chapters are easy to read and cover all of the basics. If you are looking for somewhere to start - then start here, you can find it used from about $12 on Amazon which is a real bargain!
"Electromyography for Experimentalists" by Gerald Loeb and Carl Gans is a good read if you want to start recording EMG yourself and are plan to start from scratch - the book covers everything that an engineer needs to know to record decent EMG signals. Although the technology described in this book is somewhat out of date, the basic principles remain the same and it's an excellent reference work that's full of practical information and answers.
"Introduction to Surface Electromyography" by Jeffrey Cram and Glenn Kasman is another good book that is in print and that provides a lot of electrode placement information as well as some good discussions covering protocol for human subjects.

not a book but a good source are the tutorials on EMG provided on the DELSYS homepage. They cover the basics as well as signal processing issues and some examples of application. NORAXON also provides some good to understand educational material but in contrast to DELSYS you need to leave your contact details before you can download.
All the other sources I know have already been mentioned.
